We used this app.
But then we realized it redirects users to malware / virus sites.
it was hard to notice as it only happens once per device. DO NOT USE THIS APP

Fantastic customer service. Quick response to technical issues. Easy to use. Able to direct queries to different e-mails depending on the different categories of enquiries (i.e. jobs, shipment tracking issues)
I have been looking for an app that will allow me to add data validation. And this app is perfect! it has all the flexibility I need. Their customer service is top! Never made me wait for an hour to get a response.

An easy to use way to build custom forms for your shopify pages. The support was amazing. We contacted and got an instant response to our query and the team worked with us for over an hour. Top job guys, highly recommend.
Great plugin that does exactly what we need and more. Support was very helpful in guiding set-up. A+
Great and quick support.
Really nice app and the customer support has been over and above.
Thank you!
Very happy with the quick customer service, I worked with Chirag Patel and he helped me get the formatting correct. The form customization is very good, with logic flows too. I only had trouble because the form stretched across the full width and I wanted it to match the rest of my website that wasn't full width. Chirag was quick and efficient to resolve this!
App is very simple to use. I just needed to add an extra form. Within minutes it was done. Easy as pie. Implementing it also seems very easy but I opted for a bit of help which was also done in literally less than two minutes. Thanks guys for making life easy.
