Hulk Form Builder

Danna was super kind and helpful!

The app is pretty simple to use, and has provided exactly what i wanted which was a space for customers to upload images via a form. I had a small issue which was rapidly fixed by Adna from the team via the chat box! Very satisfied with this product.

Fantastic support & smooth setup!
I’ve been using the Hulk Form Builder for my Shopify store and needed help connecting a custom “Barter This Item” button. Chirag and the Hulk team were amazing — they quickly created a backup theme, fixed the issue, and even provided clear video instructions. The button now works perfectly on my new theme, and I couldn’t be happier!

The support was professional and friendly. I really appreciate the extra care they took to make sure everything was tested and working smoothly. Highly recommend both the app and their support team.

As a small business owner working on a unique barter-based platform, having reliable tools and support like this truly makes all the difference. Thank you, Hulk team, for your amazing service and for being part of ZWAP’s journey.

This app is amazing for all types of forms. I used it on my website for sign up ups, interest forms, and surveys. Literally 3 pages of forms and I have no issues at all, just great results and easy to use forms. On top of that, they are easy to design so that they can match the website layout.

UPDATE from ~6 months in: Still 5/5 stars. I've reached out to support for the odd question here and there, and every time I've gotten a response within *minutes*. And it's not just an auto-responder. It's a knowledgable, helpful person every time. Can't say enough good things about this app.

Original review: I've just set up my first form with the Hulk Forms app, and I have to say it's awesome! I tried it as I needed a form where my customers could upload images (how is that not a built-in feature of Shopify?!) and I've been blown away with the level of options. Whatever your use case, this app can definitely handle (but you may need to be at a paid plan for some of the customizations you need).

The only feedback I have is: I wish you could make a section conditional on multiple options (so if they selected option 1, 2, or 4, show the image upload button). Luckily you can easily create multiple conditional sections, so it's easy to accomplish, but it would help keep the forms as clean as possible - rather than having duplicate sections.

5/5 stars!

Saro was excellent in helping me and my issue, highly recommend.
