Hulk Form Builder

This is a great form app for Shopify! Easy to use and very intuitive. You can create sophisticated forms with drop downs, radio buttons, and multiple choice. Easy autoresponder integration. Easy to adjust look and feel. I can't believe its free. Thank you HulkApps!

Cab dio sine things for free but they give many reasons to get you to pay the $10 per month The bad news is that if you upgrade while doing a form ( whici it provides you the option to do you lose the work that you have done. Be very careful and assuem that this is $10 per month if you do anything useful

Got stuck with the app, user error. Went on chat, Kenan answered promptly and gave me one of the fasted solutions I've ever experience using chat support, and I have to get in touch with Facebook support for ads management (and they are terrible!) And all this on a Saturday night in the UK, thank you Kenan.

Amazing service, and has exactly what i am after, customer service is great too, i had many queries and request so i jumped onto the chat service where i was connected to Amit, and he happily helped me with my many queries and request, i cannot thank you enough!! Thank you for always providing excellent service

We use the form builder app on many pages on the store. It is very useful for B2B registration, custom work forms and much more. The custom CSS block makes all the difference for us. We can use it to share more information with our customer based on the options they select. The support team does a very good job too.

The forms look neat and professional, and I love that I can receive form submissions directly to email. The customer support team at HulkApps is top-notch and always available to help with any questions or concerns. Recommend this app to anyone looking for an affordable and efficient way to create forms for their online store.

Hulk Form Builder is an awesome app for anyone that is looking to add a form to their Shopify website. It is very easy to use and offers a range of customisation options so you can tailor your forms to your store's design. The support team are also very helpful and respond very quickly. I would highly recommend this app. 10/10.

GUI is nice and integrates nicely with Shopify.. but the app is a bit buggy... the file and image upload forms failed to capture the image or file data in the form and wouldn't forward to the capture point aka the email inbox when the form was completed and submitted... opened ticket to try to resolve and exploring other form options

Super awesome app with a lot of capabilities. I love what the app offers and the conditional logic you can apply to the forms you create. They have a support chat box and their team is super responsive, knowledgeable, and helpful. Their support has helped me with anything from simple questions to more complex issues. Highly recommend.

We have a lot of B2B customers who like to have bulk orders and special pricing quoted. We used to have software that did pretty much what Form Builder did but was 10x more expensive. Once we switched to Form Builder, our customer satisfaction and conversions of prospects to sales increased. The development support is fantastic, also!
