A Very useful and easy-to-use form builder. Setting up forms is straightforward, and there are plenty of customization options available, The support team is also responsive and helpful.

My forms stopped sending me email notifications that a form had been submitted. I emailed their support multiple times on support@myappgurus.com and never got any response. I also used their chat option to leave a message saying to email me and also never got any response. I don't like to leave 1-star reviews, but I am fed up with the lack of support.
