Perfect Forms lets you build forms with a drag-and-drop editor and add them to any page or popup from the theme editor. Choose a form type: customer registration, B2B wholesale application or contact. Each answer can be saved to a customer, company, address or metafield. Review wholesale applications, then create the company, location and main contact when you approve. Add conditional fields, file uploads and address checks, and export submissions as CSV.
