Fantastic support & smooth setup!
I’ve been using the Hulk Form Builder for my Shopify store and needed help connecting a custom “Barter This Item” button. Chirag and the Hulk team were amazing — they quickly created a backup theme, fixed the issue, and even provided clear video instructions. The button now works perfectly on my new theme, and I couldn’t be happier!

This app is amazing for all types of forms. I used it on my website for sign up ups, interest forms, and surveys. Literally 3 pages of forms and I have no issues at all, just great results and easy to use forms. On top of that, they are easy to design so that they can match the website layout.
UPDATE from ~6 months in: Still 5/5 stars. I've reached out to support for the odd question here and there, and every time I've gotten a response within *minutes*. And it's not just an auto-responder. It's a knowledgable, helpful person every time. Can't say enough good things about this app.
Original review: I've just set up my first form with the Hulk Forms app, and I have to say it's awesome! I tried it as I needed a form where my customers could upload images (how is that not a built-in feature of Shopify?!) and I've been blown away with the level of options. Whatever your use case, this app can definitely handle (but you may need to be at a paid plan for some of the customizations you need).
The only feedback I have is: I wish you could make a section conditional on multiple options (so if they selected option 1, 2, or 4, show the image upload button). Luckily you can easily create multiple conditional sections, so it's easy to accomplish, but it would help keep the forms as clean as possible - rather than having duplicate sections.
