Merchants say this app is easy to set up and intuitive, even for non‑technical staff. It’s used for many form types—contact, registrations, quotes, surveys, and product requests—with strong customization like hidden fields, autoresponder emails, and workflows. Forms look professional and embed smoothly. Many report reliable performance over years and multiple stores, and describe support as fast, patient, and proactive with custom solutions and theme fixes.

Best of luck with your business! We look forward to seeing your continued success.
We had a developer add a form for us & she chose this app- so I can't say how easy it was to set up etc but it does everything I need it to. Love having an image upload, I'm sure we have received more orders because of it.
Have since made another form personally, had issues integrating it & Vatsal was excellent in sorting it out.
The app looks great on your store! Thanks so much for the review, we're eternally grateful to have you as a part of our Hulk family!
Very capable Form Builder app. Easily the most flexible and powerful form builder in the Shopify App store. Free level is good but very limited in features. My only complaint is that we signed up for Pro level - and even at that level there are some email design features that they block out unless you go to the very top level. That is in my opinion very unfair and a trap for young players.
Hello! It's fantastic to know that you find our Hulk Form Builder flexible and powerful. Your compliment really inspires us.
However, we sincerely apologize for any inconvenience you have experienced due to the email design features access at the Pro level. We aim to be transparent in our subscription level features and we regret any misunderstanding.
We'd love to learn more about your specific needs regarding email design features and ensure your concerns are directly addressed. We genuinely hear your feedback and we strive to improve our services according to our user's needs and expectations.
Your feedback and satisfaction are of utmost importance to us. Please reach out to our support team through the Shopify app for further assistance and clarifications. We assure you that our support team will be right on it.
Thank you once again for your time and patience.
Best regards,
