Merchants describe this as an easy, intuitive way to build both simple contact forms and complex, multi-step workflows with conditional fields. It’s used for RSVPs, applications, support, surveys, registrations, and payment forms, with flexible embedding and design customization. Integrations with Shopify, Google Sheets, Zapier, and cart workflows are praised, and many highlight fast, knowledgeable live chat support and strong value even on the free plan.

While I've only it used to create a couple of forms, the experience so far has been very smooth and seamless. I really look forward to diving more into this product. *Stay tuned for an updated review!*
I have used this app for a couple of years now and love it! Love how I can customize my forms to collect specific information based on what I need. Tech support is quick to answer any questions I have.
We're really grateful and appreciate you taking the time to share your rating with us 💜.
I used this app to add a trade application form to my website. Very easy to do and it looks great. I use shopify and the form integrates with one click no need to write any code. Brilliant idea and free.
I really like this plug-in and it really helped me get the job done fast. Only one complaint: I wish I could re-arrange some elements (left/right) rather than just everything stacking up on top of each other
I used the app to use a form where visitors can leave their name and email address, so I can send them personalized emails. It's very user friendly, easy to handle and a piece of cake to install on shopify
Excellent app with excellent support. Special shoutout to Richarde M. who was extremely helpful in supporting me through my first steps. He is the best. This is the best form builder I have experimented with.

I was very pleased with the Form Builder App. It was very easy to use and very easy to add it to my site.
Creating the form was simple and very intuitive. I would highly recommend using this App for your store.
I used this app to build a dealer/distributor sign up form. Very easy to use and fairly user friendly. Definitely the right price at FREE. Would like to see more control over font sizes. Color picker is excellent.
Process took maybe ten minutes. The form was just what I needed. Awesome that the free membership gives you what you need to create the form. If my business grows, the paid membership will definitely be in the future.
