Merchants describe this as an easy, intuitive way to build both simple contact forms and complex, multi-step workflows with conditional fields. It’s used for RSVPs, applications, support, surveys, registrations, and payment forms, with flexible embedding and design customization. Integrations with Shopify, Google Sheets, Zapier, and cart workflows are praised, and many highlight fast, knowledgeable live chat support and strong value even on the free plan.

Great support staff. A great number of valuable plug-ins to supplement my online store and the foundation that Shopify provides. Definitely recommend!
I created a form that would allow business reps to have a more customized shopping experience and capture contacts. Very easy, and best of all it was free.
It still need some more additional features but so far so good.I like this for what I need. I use cognito and I may be able to do everything from this app now
This app is exactly what I needed! It is very user friendly and it made creating the form that I needed a breeze. I had a question and support was right there to answer.
I used this app to create a booking form for workshops in my store. Ii is great how customizable it is. I wish I could add different payment options for different bookings.
Excelente, Me gusto que lo pude elaborar fácilmente y es completamente editable, aparte de que tiene la función que buscaba para que los visitantes puedan subir un archivo o fotografía
I found it really easy to use. Got my custom form up and running in a matter of minutes. I would definitely recommend this to anyone else who need to make a custom form and not do any coding.
