The app is a paid version and needs to be subscribed to work in the store. You have not subscribed, so the options you created do not appear in the store.
Please rest assured that the app has a 14-day free trial. For example, if you install and subscribe to the app on the 1st, the subscription period will start from the 15th. The 14 days from the 1st to the 14th are free trials. Uninstalling during the trial period will not incur any fees. This is also the logic of most apps for free trials and is the free trial solution provided by Shopify for apps.

Great app that adds a bunch of customizations
Shopify Variant options were not enough for the highly customizable products I sell. Ymq Product Options solved the entire issue at a very fair price.
Option Templates are the perfect solution for my shop.
Their customer service is quick, with clear instructions or links to help you fix the issue at hand.
Would recommend to anyone with highly customizable items for sale.
I tried many different apps with similar functions however Ymq Product Options was the most user-friendly and straightforward app to use.
