It’s used for many form types—contact, registrations, quotes, surveys, and product requests—with strong customization like hidden fields, autoresponder emails, and workflows. So far so good! Had an issue with theme and page integration, Cristian gave me and my business TOP TIER support, solved a problem that was specific to my theme, and all is working as it should now.
I download very few apps to keep my business lean and mean, HULK will be staying on my site for the long haul!

The form app is good for a free tier version, one thing to improve is when the page loads, the form loads in delay which cause in layout shift.
Hulk Form builder is a good match with Shopify and meets all our requiremnts.
Thanks for solving the prblm quickly
So far, so good. I highly recommend it!
I wanted something a little different for my form. I wanted the form to be in a Collapsible Row on a product page template. Support from Hulk was able to make that happen quickly and at no additional charge. We've just started using the form feature, but so far it's great.
Thank you Hulk Contact Form Builder. The form stopped working after new theme version updates. I had immediate response on "Live Chat" and the form issue was resolved successfully. Thank you very much Ken. I am very small business and so I love this free form very much to support my business queries/booking.
I hate this app. While editing it suddenly reverts your changes. Even after i pressed saved it removed all my changes after a couple of minutes. If you're further down while editing and scroll up then it reverts your changes. It's impossible to create a form with this app!
thanks,Vatsal!!
